MEDICAL REQUIREMENTS

 

UPLOADING MEDICAL DOCUMENTATION PROCESS

Profiles/MEB/NDDE's : If you are submitting documentation for a Profile (469), MEB or NDDES case it will need to be submitted via the MHS Genesis Patient Portal using the following steps: 

1. Visit https://patientportal.mhsgenesis.health.mil/ 

2. Login with your CAC

3. Select Messaging at the top, then click send message. 

4. In the "To" field, type "113" and Air Nat'l Guad 113 MDG Readiness should automatically pop up, select that. 

5. Attach your files to the email and hit send.  

This feature automatically uploads the documents to your Military records and notifies the MDG to reviewAfterwards, the MDG will require 10 business days from date verifying that all documentation required is complete and sufficient--- to complete profile.  All profile requests, at minimum, will require a signed and dated letter on letterhead from provider with circumstances to injury/illness, diagnosis, restrictions, duration of restrictions and prognosis. 

**Note**: We cannot create profiles short notice or the day of your Fitness Test so please plan accordingly! Incomplete or missing profile documentation will be returned without action (RWOA). 

These lists are current as of 7 March 2025. All Airmen Engineers should always check their Individual Medical Readiness (IMR) during each RSD. If a dental medical requirement has been completed by a civilian provider, i.e., DD Form 2813 signed by a civilian dental provider, please email the form to 113.WG.Medical.Documents.Org@us.af.mil.

Sunday | Malcom Grow Medical Center (MGMC)

RSD on Sunday from 0730-1000 for appointments (if you're not 15 minutes early, you're 15 minutes late)

Note, appointments will only be accepted through COB Thursday before the RSD. No patients will be added to the appointment schedule on Saturday.

If you have already received the vaccine from a civilian pharmacy, please submit a copy of your vaccination via the MHS Patient Portal using the following instructions:

Mental Health Assessment (MHA) Procedure:

Option 1: MSgt Whalen sets up a schedule with Ms. Wolff during RSD. For missed appointment, proceed to Option 2.                                                                                                                        Option 2: Schedule an appointment with Ms. Wolff during the week by phone Comm: 240.857.6442, Cell: 240.432.1166, or email at paula.wolff@us.af.mil.

PHAQ Procedure

1. Log into the Air Force Portal Homepage at https://www.my.af.mil/

2. Under Quick Links, type “MyIMR” and select MyIMR/ASIMS-Medical Readiness  

3. Click on “PHAQ” under IMR Action List

4. Answer PHAQ questions. Enter personal phone and email. MHA will be calling this number.

5. When finished, wait 15 minutes, then complete MHA portion. Instructions above.


Medical personnel will still need review and close out the PHA before you are IMR Green.
